Grantee Requirements
The Home Depot Foundation requires all grantees to submit a final project report. In certain situations, grantees may also be required to submit progress reports in addition to the final project report.
If you would like to submit a Success Story of a project supported by The
Home Depot Foundation, please utilize our Success Story Template and submit via email to hd_foundation@homedepot.com.
Direct Grant Recipients
If you received a Direct Grant from The Home Depot Foundation, please use the following link to submit your final project evaluation. If you are required to submit progress or interim reports, please use the below link as well.

Pass-through Grant Recipients
If you received a Pass-through Grant from one of The Home Depot Foundation’s national partners, you are required to complete and submit a Pass-through Grant Recipient Fact Sheet.
